Please call the office several days to a week before your prescription expires. Although we send prescriptions electronically, we may need additional time to get prior authorizations or review clinical information.

We respond to most inquiries within 24 hours but no later than 2 business days. Please leave detailed information on how and when to contact you.

Patients can complete intake forms upon arrival or print and bring the completed Registration Form with them. Find the link to the forms here.

You can expect a thorough and detailed evaluation including a history and physical examination. Additional testing such as blood work, imaging and pulmonary function tests may be recommended as needed.

Consultations typically last between 45 minutes to an hour. We want to take the time to fully address and understand your concerns. Every patient is treated with the same care and attention. Please be patient if this takes more time than anticipated.

  2. Valid ID or Driver’s License
  3. List of current medications
  4. Referral from Primary Care Provider (if required by insurance)
  5. Relevant labs, imaging reports ( i.e. CT scan, chest xray, PET scan), clinical Notes from other physicians.
  6. Co-pay or Self-pay fee
  7. Registration Form (This can be filled out in the office as well.)
